Assisting Hands Home Care Wilmington Caregivers Will:
- Coordinate with the discharge team to ensure proper follow-up
- Do a home safety evaluation
- Ensure prescriptions are provided and acquire new prescriptions
- Provide medication reminders and remove old or unnecessary meds
- Schedule follow-up doctor appointments and help your loved one get to them
- Assist with the use of new equipment (wheelchair, walker, Hoyer, etc.)
- Prevent falls and provide awareness and education
- Help with illness/recovery-specific education
